Student Media Consent and Release Form

Throughout the program duration, students may be highlighted in efforts to promote FOCUSTUDIO activities and achievements.  For example, students may be featured in materials to train professors and/or increase public awareness of our Academy through newspapers, web, brochures, and other types of media.

In consideration of my engagement as an active student of FOCUSTUDIO hereby grant FOCUSTUDIO, LLC (“Academy of Photography”), his/heirs and their legal representatives and assigns, directors and staff, permission to photograph and/or video recording me during the classes/workshops/trips or any other type of event the Academy organize.  This includes all videorecording of classes by Skype, Zoom or any other conference call platform. 

They have the irrevocable, perpetual and unrestricted right and permission to take, copyright and use, re-use, publish and republish, separately or in conjunction with other photographers, photographic portraits/pictures or videos of me or in which I may be included, in whole or in part,  composite or distorted in character or form, without restrictions to changes or alterations, in conjunction with my own or a fictitious name, or reproductions thereof in color or otherwise, made through any medium, and in any and all media now or hereafter known, specifically including but not limited to print media and distribution over the internet (Instagram, Facebook, web pages, Twitter, etc.) for illustration, promotion, art, editorial, advertising, or trade. 

I specifically consent without restriction any changes or alterations to the portraits/pictures or videos as to color, size, shape, perspective, context, foreground or background.  I also consent to the use of any published matter in conjunction with such photographs. 

I hereby waive any right that I may have to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection with them or the use to which they may be applied.  I hereby release, discharge and agree to hold harmless the Focustudio LLC. (“Academy of Photography”), his/her heirs, legal representatives and assigns, directors or staff, and all persons acting under his/her permission or authority or those for whom he/she is acting, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of such photographs/videos or in any subsequent processing of them, as well as any publication of them, including without limitation any claims for libel or violation of any right of publicity or privacy.

I am also fully aware that I will not receive any monetary compensation or any other type of compensation for my participation on those photographic portraits/pictures or videos.

Student Photo Release Form

In consideration of my engagement as an active student of FOCUSTUDIO hereby grant FOCUSTUDIO, LLC (“Academy of Photography”), his/heirs and their legal representatives and assigns, directors and staff, permission to use and publish any of my photographs and/or video recording taken by me during the classes/workshops/trips or any other type of event the Academy organize and in which I may be enrolled. 

They have the irrevocable, perpetual and unrestricted right and permission to take, copyright and use, re-use, publish and republish, separately or in conjunction with other photographers, any of those photos/pictures or videos mentioned above, in whole or in part,  composite or distorted in character or form, without restrictions to changes or alterations, or reproductions thereof in color or otherwise, made through any medium, and in any and all media now or hereafter known, specifically including but not limited to print media and distribution over the internet (Instagram, Facebook, web pages, Twitter, etc.) for illustration, promotion, art, editorial, advertising, or trade.

Photographic Exterior & Studio Practices Acknowledgment / Liability Release Form

    In consideration of my engagement as an active student of FOCUSTUDIO, LLC (“Academy of Photography”), I understand that participating in any Photographic Exterior Practices / Studio Practices could involve risk of physical injury, illness, death or property loss, and despite any safety precautions, FOCUSTUDIO, LLC cannot guarantee safety thereof, as all risks cannot be prevented.

Therefore, I, do hereby agree to assume all risks and responsibilities surrounding my participation in any  Photographic Exterior Practices / Studio Practices, and I do hereby release, indemnify, and hold harmless FOCUSTUDIO, LLC., Officers, Directors, Professors, Employees, Staff, Volunteers, representatives, assigns and any participants from and against any and all liabilities to the undersigned, for any and all physical or psychological injury to my person or property, including but not limited to illness, paralysis, death, damages, economical or emotional loss, expenses (including attorney fees) claims, property loss, medical expenses, personal expenditures, judgments, actions or causes of action as a direct result of my participation on any Photographic Exterior Practices / Studio Practices, including traveling and transportation to and from an event related to this Photographic Exterior Practices / Studio Practices whether caused by negligence of FocuStudio, LLC, of persons acting on its behalf or otherwise.

I further acknowledge that FocuStudio, LLC, does not provide any type of transportation to, from and during the entire length of the Photographic Exterior Practices / Studio Practices and that such transportation will be my responsibility.

I acknowledge that I have carefully read and fully understand this “waiver and release” and fully understand that it is a release of liability.  I expressly agree to release and discharge FOCUSTUDIO, LLC., and all of its affiliates, managers, directors, members, staff, professors, volunteers, representatives, and assigns, from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against FOCUSTUDIO, LLC for personal injury or property damage, and I voluntarily sign this document in order to participate in any Photographic Exterior Practices / Studio Practices during the entire length of my studies at FocuStudio, LLC

In the event that any damage to equipment or facilities occurs as a result of my willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any actions of neglect or recklessness.

COVID-19 Student Release Form

In consideration of my engagement as an active student of FOCUSTUDIO, LLC (“Academy of Photography”), I understand that at any symptom I may have of COVID-19, is my responsibility to communicate with FocuStudio Directors, and I should not attend any exterior/studio practice, workshop, or any other activity that may involve my personal contact with any other member of FocuStudio (student, professor, staff or models), in order to limit community, spread of the illness. And in case that I began having the symptoms or the results of a test come positive after an exterior/studio practice, etc., is my responsibility to communicate with FocuStudio Directors so they can communicate with the individuals that I may have been exposed or interact during such events, before knowing such results or having those symptoms.

I acknowledge and understand that I am the only individual capable of determining if I am experiencing COVID-19 symptoms.  I hereby agree and willingly assume responsibility for any risks that I expose myself and accept full responsibility of getting the disease or death that may result from participating in any activity (exterior/studio practice, workshops, etc.) that FocuStudio, LLC may organize.  By signing this form, I agree to waive and to not hold FocuStudio, LLC or any of his Directors, Professors or Staff for any sort of virus COVID-19 or illness that I can get while attending or training during exterior/studio practices, workshops, etc. that FocuStudio, LLC may organize.  This Release will be valid for the period of time I am enroll / participating in FocuStudio, LLC courses, workshops or any other event.

I agreed that during the entire duration of the exterior practice, workshop, studio practice, etc., I should wear a mask, use sanitizing gel, and practice social distancing.  It is not responsibility of FocuStudio, LLC to provide such items.

I have read this release and understand that I am participating in all FocuStudio, LLC activities at my own risk.
